In Legistar there is a situation where when you run your agenda or minutes report that an agenda item may be missing from the report although the item is on the agenda or minutes. The issue is that there are duplicate line numbers and the report will only show the first item. To fix the issue you need to go to Tools and select Fix Line Numbers.
In the image below you can see that line 5 appears twice.
